The reason it works is grade, not speed. Using the standard ACSM walking equation, 3 mph on flat ground sits near 3.3 METs. The same 3 mph at 12 percent grade lands near 8.3 METs, roughly two and a half times the metabolic cost. That is a walking pace in territory most people can otherwise only reach by jogging, without the ground impact.

The handrail problem

This is the most common mistake, and it is not a small one. Research in the International Journal of Exercise Science found that handrail use during inclined treadmill walking cut metabolic cost by roughly a third when subjects leaned back on the rails, while light upright contact produced no significant reduction at a 10 percent grade.

Loaded incline walking and rucking share a lot of machinery. Both are sustained aerobic work at a walking cadence with load on the torso, and both hammer the calves, soleus, glutes, and hamstrings in the endurance pattern a long march demands. Thirty minutes at 12 percent under a vest is legitimate preparation for the aerobic side of a ruck standard.

Be honest about the limits. A treadmill removes what actually breaks people on rucks: uneven footing, downhill eccentric loading, weather, pack fit, and hot spots. Incline work builds the engine, not ruck-specific tolerance, and it will not toughen your feet.
